Demographic Information

That information also allows us to determine if this Website is working properly, make changes to the Website that allows users to have better experiences on the Website, and improve the Website, our offers and promotional content so that they are more relevant to our visitors. This information may be shared with fulfillment partners and other third parties on an aggregated basis, but it does not include the Personal Information of individual users (defined below). 

  • Pixel Tags, also known as clear GIF or web beacon, is an invisible tag placed on certain pages of the Website but not on your device. When you access these pages, pixel tags generate a generic notic of that visit. They usually work when a particular device visits a particular page. The Island Spine Center uses pixel tags for website analytics to serve you more relevant content and promotional offers, when applicable, based on your historical behavioral patterns. 
     

  • Your network service provider normally assigns a dynamic IP address to you every time you use your mobile browser. We collect your mobile device’s IP address in order to conduct system administration, Mobile Site analysis, block devices that are intending to hack or otherwise damage our Mobile Site, and identify devices which may be engaged in illegal or illicit activity on our Mobile Site. While we do not associate IP addresses with records containing Personal Information, we will use IP addresses to identify any user who engages in misconduct or otherwise violates any applicable laws. Additionally, we may be required to disclose IP addresses pursuant to a court order or subpoena. Island Spine may also identify and track your Internet protocol (“IP”) address. An IP address is the unique number assigned to your server or internet service provider (“ISP”). Island Spine may track those IP addresses for system administration purposes, to report aggregate information, site tracking, security purposes, or to prevent our data and servers from being damaged.

 


Personal Information

NOTE: This Website is not intended for children. We do not knowingly collect, use or disclose Personal Information about visitors under 13 years of age. For more information, please see Section 10, “What About Children,” below.


At its option, Island Spine Center may collect personally identifiable information from you when you use some of the features on the Website., when you sign up to receive a free download, when you provide survey information, or when you sign up to receive any other feature we may offer in the future which requests information from you.  The Island Spine Center stores your Personal Information on its servers and/or may share and store such Personal Information with third party subcontractors, affiliates of Island Spine Center,  fulfillment partners, and marketing partners.


Personal Information may include: your first and last name, age, your mailing address, your billing address,  email address, telephone number, cell phone number, date of birth, credit card information and related financial information, survey information concerning the services or products you are interested in based upon your survey data, information concerning your interests, occupation, product uses and preferences, and other demographic information that could help identify you. It will however not identify any prognoses, diagnoses nor medical information about you as per our requirements to HIPAA.


If you have set up an account that is password protected in order to use any booking features on the Website, order products, or for any other purpose for which a password is needed, that password is also part of your Personal Information. 


It is possible to use portions of this Website without providing any Personal Information, but it may be necessary to provide Personal Information to utilize all features of this Website.

 

 

2.  How Secure is Your Personal Information?

Island Spine is not responsible for any breaches of a user’s profile and access to their Personal Information which is attributed to lost or stolen user-names and passwords. In the event of an inadvertent disclosure of a user’s user name and password, users are advised to change their password as soon as they become aware of such disclosure.  Island Spine will also reach out to any affected users if personal information has been breached or disclosed as per our HIPAA requirements, as stated in our HIPAA Compliance Policy.

We will take reasonable measures to have those third parties maintain the confidentiality, security, and integrity of the Personal Information they obtain from us.

 

We reserve the right to disclose your Personal Information as required or permitted by law and in accordance with HIPAA compliance regulations.  For example, we may disclose it to respond to a subpoena, court order or other legal process, or in special cases when we have reason to believe that disclosing Personal Information is necessary to identify, contact, or bring legal action against you if we  believe that you may be violating your agreements with us (including, without limitation, any applicable terms of use), or may be causing injury to, or interference with, Island Spine Center MV, any other user of this Website or any service, the general public or other third parties.

This Website is designed by the Island Spine Center MV and intended to be considered as a general audience Website and is not intended for use by children.
